I've worked in a transmission shop (and dealers), and over the years I've seen mfg try and address the shift fork issue and all failing. The plain fork is often hard, as is the dog-clutch that it moves. Constant pressure like that Scott referred too, means something is going to wear. I've seen phosphor-bronze used, and later teflon and other "plastic" dips tried. The end result is always the same. If the fork is softer than the clutch, metal and dirt particles get imbedded in the softer material and act like a grind stone on the harder. If both pieces are hard, that doesn't happen, but if improper adjustment of the linkage or operator error puts a constant load on the moving pieces things wear! End result there is nothing that works 100% of the time.
I remember replacing all shift forks in GM's front wheel drive transmissions for that reason. I don't recall it helped much, but got the cars to the point they were out of warrantee.
